Sending Us Your Material
With the exception of the Personal
Profile, which should be submitted electronically, please send
all of the material for your Waiting Parent Web Site Package by mail together
to:
Canada Adopts!
550 Eglinton Ave West
Suite Box 23052
Toronto ON
M5N 3A8
Please note: Do not send your material
by courrier. If you would like to send it by express mail, please use
Canada Post Priority Post.
Here's
a Checklist of Everything You'll Need to Send Us:
- Copy of the front page of your
home study or letter from your adoption practitioner or agency on official
letterhead stating you completed a home study.
- Signed copy of the Terms
of Use and Disclaimer. Please print out a copy of the last
page, sign it, date it and include it with your application. If you're
a couple, both partners will need to sign it.
- "Dear Birth Mother"
letter. An autobiographical profile of about
500 - 1000 words written by you describing your life and the kind of
life you hope to provide for your child. Please make it available on
a CD, in either a Microsoft Word or text (.txt) file. Do not format
your letter, except for paragraphs, and avoid the use of fancy fonts
or typefaces. For tips on how to write an effective letter, click here.
- Two colour photos.
Be sure that the photos you send are originals, with high contrast.
Reproductions and photocopies suffer from bad quality and lack crispness.
You can also send us digital photos on a CD. Make sure that digital
photos are large (at least 4 by 6 inches) and saved in .jpg format. Please do not send digital photos embedded in Microsoft Word or Power Point files. If you're a couple, at least
one photo should be a close up of both of you. If necessary, we may
have to crop your photo slightly so that it fits into our format. One photo will appear on your "Dear
Birth Mother" letter; the other with your Personal Profile. For
tips on how to choose an effective photo, click here.
- Personal Profile.
The results of a short autobiographical survey which will be included
as the second page to your site. Once you've completed it, submit it
directly to us electronically. To fill out the form, please click here
.
- Short biographical description.
25 - 40 words about you or your reasons for adopting to be included
in your listing next to your thumbnail photo.
- E-mail Address and Phone Number.
Electronic address and phone number where birth mothers can reach you
or your adoption professional directly. Please include these as either a Microsoft Word or text (.txt) file. Do not send hard copies as this contact information is a very important part of your profile as this is the only way birth parents can contact you. Although we try to be very careful, errors occur when email addresses and phone numbers need to be re-typed. Also, be sure to include an
e-mail address or phone number where we can reach you if we have any
questions—that is, if it's different from the ones listed on your
web site. If you don't have an e-mail address you can set up a free
account through a number of web services such as Hotmail, Yahoo or Gmail.
- Self-addressed stamped envelope. If you send originals, be sure to include a self-addressed stamped
envelope so that we can return them after we've finished. We will not return CDs unless you provide an envelope large enough to fit the CD as well as suffiecient postage.
- Cheque payable to "Canada
Adopts!"
